Lets Discuss Health Inequalities

Spotlight on Improving Health Inequalities: What is CORE20PLUS5?

In the 2024/2025 PCN DES service requirements, the focus on Improving Health Outcomes and Reducing Health Inequalities shines a light on the importance of the CORE20PLUS5 approach. But what exactly is it?

CORE20PLUS5 is an NHS initiative designed to tackle health inequalities both nationally and within local systems. It targets specific population groups, including the most deprived 20% of the population and inclusion health groups such as ethnic minorities, individuals with learning disabilities, coastal communities, young carers, and sex workers. Additionally, it hones in on five key clinical areas in need of rapid improvement.

At DKJ, we’re actively engaged in various projects aimed at reducing health disparities across the country.

Recently, we established a Drop-In Session specifically for Ethnic Minority Women. Recognizing that they weren’t accessing healthcare services as much as needed, we collaborated with community leaders to create a safe space where these women could receive health and care information. The Drop-In, initially run by volunteers eager to enhance their skills for future employment, offers a range of services including GP practices, sexual health services, doula support, immigration advice, and connections to mental health practitioners and social prescribers. Interpreters are also available to provide vital healthcare guidance and support.

Our efforts have yielded significant results, with over 30 women attending the Drop-In weekly. This initiative ensures essential healthcare support reaches the heart of the community, where it’s needed most.
