The Fuller Review states: “Integrated neighbourhood ‘teams of teams’ need to evolve from primary care networks (PCNs) and be rooted in a sense of shared ownership for improving the health and wellbeing of the population”.

So that’s PCN teams, wider primary care providers, secondary and social care teams plus other health and social care partners such as domiciliary care and the voluntary sector, all working together to form MDTs that can improve the health and wellbeing of their patient population together.
