Deep Dive: The 10 Year Plan and What Should PCN's Be Doing Now

Due to high demand, join us for a DKJ Deep Dive exploring the Government’s new 10-year health plan, which sets out a radical redesign of general practice.

From 2026, two new contracts will be introduced to encourage GPs to work at scale and lead new ‘neighbourhood health services’ aimed at shifting care out of hospitals and into the community:

Neighbourhood contract – for services covering around 50,000 people, likely aligned to PCN footprints.
Larger provider contract – for those covering populations of 250,000 or more, with broader responsibilities including taking on struggling GP practices.

At the centre of this new model are neighbourhood health centres, open 12 hours a day, six days a week, and bringing together expanded multi-professional teams under one roof – from GPs and nurses to social care staff and employment advisers.

We will also provide an overview of the CQC implications for practices and PCNs coming from the 10 Year Plan.

We’ll unpack what this means for GPs, explore how these models might work alongside the traditional partnership model, and discuss what actions practices, PCNs, and GP Federations should consider now.

Understanding Regulated Activities: What You Need to Know

If you provide certain types of care or treatment in England, you may be carrying out a regulated activity. These are services that can affect someone’s health, safety, or wellbeing – and by law, you must register with the Care Quality Commission (CQC) to offer them.

What counts as a regulated activity?
Common examples include personal care, nursing care, GP and dental services, surgical procedures, and remote medical advice. A full list is available on the CQC website.

Why does it matter?
Regulated activities must meet strict quality and safety standards. Operating without registration is illegal and could lead to fines or prosecution.

Who needs to register?
Any individual, partnership, or organisation providing regulated services – from care homes and home care providers to private clinics and online health services. If you’re running the service day to day, you may also need to register as a Registered Manager. Larger organisations must appoint a Nominated Individual to act as the main contact with the CQC.

How do I register?
You’ll need to show relevant experience, complete a DBS check, and provide details of how you’ll manage care safely. The CQC may also interview you as part of the process.

Who checks the CQC?
The CQC is independent but reports to Parliament and is held accountable by bodies like the Department of Health and Social Care and the National Audit Office.
